NOTICE OF PRIVACY PRACTICES
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
At Medetone LLC, we are required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with notice of our legal duties and privacy practices with respect to PHI. We are required to abide by the terms of this Notice of Privacy Practices.
I. USES AND DISCLOSURES OF YOUR PHI
We may use and disclose your PHI for the following purposes:

Treatment: We may use and disclose your PHI to provide you with pharmacy care, medical treatment, or services. For example, we may disclose PHI to your doctors, nurses, or other healthcare providers who are involved in your care to ensure they have the necessary information to treat you.

Payment: We may use and disclose your PHI so that the pharmacy services you receive may be billed to and payment may be collected from you, an insurance company, or a third party. For example, we may need to give your health plan information about a prescription you received so your health plan will pay us or reimburse you.

Healthcare Operations: We may use and disclose your PHI for our pharmacy operations. These uses and disclosures are necessary to run Medetone LLC and make sure that all of our patients receive quality care. For example, we may use PHI to review our treatment and services and to evaluate the performance of our staff.

Business Associates: We provide some services through contracts with business associates (e.g., software providers, accountants, or legal counsel). We require these associates to appropriately safeguard your information.
II. OTHER PERMITTED USES AND DISCLOSURES
We may also use or disclose your PHI without your authorization in the following circumstances:

III. YOUR RIGHTS REGARDING YOUR PHI
You have the following rights regarding the PHI we maintain about you:

Right to Inspect and Copy: You have the right to inspect and obtain a copy of your PHI that we maintain. We may charge a reasonable fee for the costs of copying and mailing.

Right to Amend: If you feel that PHI we have about you is incorrect or incomplete, you may ask us to amend the information.

Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we made of your PHI for purposes other than treatment, payment, or healthcare operations.

Right to Request Restrictions: You have the right to request a restriction on the PHI we use or disclose about you for treatment, payment, or healthcare operations. We are not required to agree to your request, but if we do, we will comply unless the information is needed for emergency treatment.

Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location (e.g., only at a home phone number).
IV. CHANGES TO THIS NOTICE
Medetone LLC reserves the right to change this notice. We reserve the right to make the revised or changed notice effective for PHI we already have about you as well as any information we receive in the future.
V. COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with Medetone LLC or with the Secretary of the Department of Health and Human Services. There will be no retaliation for filing a complaint.
